- When requesting and claiming the official results, the patient and authorized representative must provide a photocopy of their Government Issued or Valid ID’s.
- All documents requested are with corresponding amount to be paid at the cashier.
- Releasing of documents are only during working hours Monday to Saturday 8AM to 5PM except for Sunday and Holidays.
